Registration Information

2010 REGISTRATION DATES
Sat.      April 17th 9am-12pm Schaefer Elementary

Sat       April 24th 9am-12pm Schaefer Elementary

Sat.      May 4th 6pm-8pm Schaefer Elementary

            (Late sign-up for recreational players only)

 

If you mail in your registration after the May 4th deadline, you will need to pay an extra $10.00 per child, as a late fee. We will only accept mail-in registration forms, with all required documents (proof of age and photo, if applicable), until May 31st.

 

All Under 14, 16 and 19 players must supply a recent passport size photo (1 ½” x 1½”) with their registration to be used by the club for player passes.  A photo must be supplied or the player can not be processed.  No hats or sunglasses.

All registrations will be held at Schaefer Elementary School located at 1370 San Miguel Avenue: off Coffey Lane ( Northwest Santa Rosa)
 

FEES (before May 4th)

 $105 per child ($105 for 2nd child, $50 for 3rd child, and $50 per additional child).


FEES
(after May 4th, including late charge)

 $115 per child ($115 for 2nd child, $60 for 3rd child, and $60 per additional child).

 

ONLINE REGISTRATION FEES (from TBD to TBD)

There will be a convenience fee added for the online registrations.

The fee is yet to be determined. Credit cards only!!!

You may download the CYSA U.S. Youth Soccer Membership Form.

Print this completed form TWICE.   The parent/legal guardian/player age 18 or over must originally sign both copies in blue ink..

 

New players must submit proof of age with their Registration (birth certificate, birth registration issued by an appropriate government agency, Board of Health records, passport, alien registration card issued by the United States Government, a certificate issued by the Immigration and Naturalization Service attesting to age, or a certification of an American citizen born abroad issued by the appropriate government agency, a current driver's license, or a current identification card issued by the Department of Motor Vehicles.  Hospital, baptism, or religious certificates, Certification of Birth for school registration/employment only, affidavits, will NOT be accepted.  

 

REFUND POLICY

A complete refund, excluding the raffle ticket portion, will only be given up to May 31st, if requested in writing by this date.  Submit requests via email to info@northwestoaks.org or via mail to Northwest Oaks Soccer, 1415 Fulton Road #205-C232, Santa Rosa, CA 95403.  Refunds after May 31st will only be given for medical reasons or for relocation out of the area.  Refunds in these instances will be $76 (or $50 in the case of a family discounted fee).  No refunds will be made on raffle tickets at any date.
