If you mail in your registration after the May 10th deadline, you will need to pay an extra $10.00 per child, as a late fee. We will only accept mail-ins, with all required documents, until May 23rd.

All registrations will be held at Schaefer Elementary School located at 1370 San Miguel Avenue: off Coffey Lane ( Northwest Santa Rosa)

FEES
Registration fees are $100 per player. Fee includes $76 registration fee and a $24 dollar club fee that includes a $15 raffle/drawing fundraiser.

WHAT YOU‘LL NEED TO REGISTER A CHILD
At least one parent or guardian must be present to sign the registration form. New players or players who have not played in Northwest Oaks since 2006 must supply a birth certificate. Other acceptable Proof of Age certificates are:
Board of Health Records
Passport
Alien Registration Card issued by the United States Government
Certificate issued by the Immigration and Naturalization Service attesting to age
Certificate of an American Citizen born abroad
Current Driver’s License
Current Identification Card issued by DMV

NOT acceptable Proof of Age: Hospital Certificate, Baptism Certificate, Religious Certificate, Immunization Records, Sonoma County School/Employment only Certificates or Affidavits

All Under 14, 16 and 19 players must bring a recent passport size photo (1 ˝” x 1 ˝”) to be used by the club for player passes. If you do not bring a photo of the player, the player must accompany you to registration to have his or her photo taken. A photo must be supplied or taken the day of registration or the player can not be processed.


FOR YOUR CONVENIENCE
You may download the CYSA U.S. Youth Soccer Membership Form.  This form requires two (2) wet-ink signatures from the parent/legal guardian/player age 18 or over. The wet-ink signatures must not be in black. Please print this form twice and bring it completed to one of our registrations.  For your convenience we have also provided an editable PDF of the CYSA form.  Fill-out online and then print two copies and bring with you to registration.
