Note: This form requires two (2) wet-ink signatures from the parent/legal guardian/player age 18 or over. The wet-ink signatures must not be in black ink. Please print this form twice and return it completed to one of our scheduled registrations.

Team Official Registration & Risk Management Disclosure Form

This form is to be completed by any adult wishing to coach, assistant coach, manage or team assist any Northwest Oaks Youth Soccer team.

ACCIDENT/INJURY REPORT

In the event of an inury to a registered team player, coach, assistant coach, manager or volunteer of a team during a sponsored and supervised activity of CYSA a Youth Soccer Case Report Form must be completed.

It is the responsibility of the coach and parent to see that the Youth Soccer Case Report form is filled out properly before submitting it to CYSA. The insurance runs for the same period as the playing season, September 1 through August 31.

Brick or Star Order Form  Buy a brick or a star with your own personal message for the “Walk of Fame” at A Place to Play. A place to Play is the West Third Street Sports Complex in Santa Rosa. The complex is home to four  official size soccer fields and two baseball diamonds.
