Referee's are an important part of the game.  The referee is there to keep the game fair for the players, and to enforce the Laws of the Game.  How many times have you stood on the sidelines, or were playing in a game and found yourself saying that you could do a better job than the person out there?  Well here's your chance.

 This will take you to an online form that you can view on your computer.
I sent off a player or coach, now what do I do? Copied from the CYSA (California Youth Soccer Association) web site.

Send Off Report

The official report to use for CYSAN (youth) games.   Must be completed within 24 hours of issuance of the send off.  This can be filled out on a computer and printed. The only drawback is that the data you enter cannot be saved to your computer.  Adobe Acrobat reader required for use.

Pay Sheet

Just a simple Microsoft Excel file that can be filled out on the computer, and then printed out and/or saved for your files.  Turn this in for payment for the games you worked along with the game card.  

Game Cards

If your out of the regular game cards, you can print this out and use it instead.  Turn this in with the above pay sheet for  games worked.
Sample Pregame instruction's between Center Referee's and Assistant Referee's Sample sheet that includes most of what should be covered in a pregame dialog between a center and their assistant referee's.
